Conclusion

Spontaneous perforation of pyometra is rare but should be considered in elderly women with acute abdominal pain.

Supporting Evidence

  • The incidence of spontaneous uterine perforation due to pyometra is about 0.01% to 0.05%.
  • Only 25 cases of spontaneous perforation of pyometra have been reported in English literature.
  • The patient presented with severe abdominal pain, vomiting, and fever.
  • Laparotomy revealed about 900 mL of pus in the peritoneal cavity.
  • Histological examination confirmed the diagnosis of pyometra without malignancy.

Takeaway

Sometimes, a woman's uterus can get infected and fill with pus, which can cause it to burst. This is very rare, but it can happen, especially in older women.

Methodology

The patient underwent laparotomy after being diagnosed with gastrointestinal perforation, which revealed a perforated pyometra.

Limitations

The study is based on a single case report, limiting generalizability.

Participant Demographics

A 63-year-old Iranian woman with a history of diabetes and three normal vaginal deliveries.
