Hypothesis
Increased levels of prostanoids in bronchoalveolar lavage (BAL) fluid would predict clinical outcome in congenital diaphragmatic hernia (CDH) patients.
Conclusion
Prostanoid levels in BAL fluid do not predict clinical outcome in CDH patients.
Supporting Evidence
- Different concentrations of prostanoids were found in BAL fluid of CDH patients with persistent pulmonary hypertension.
- High levels of 6-keto-PGF1a and TxB2 were associated with poor prognosis in some CDH patients.
- TxB2 levels showed large variability in all CDH patients irrespective of outcome.
- Prostanoid levels in BAL fluid did not correlate with ventilatory parameters.
Takeaway
Doctors looked at special substances in the lungs of babies with a breathing problem to see if they could tell who would get better, but they found it didn't help.
Methodology
The study measured concentrations of prostanoids and various inflammation markers in BAL fluid from 18 CDH patients and 13 control subjects.
Limitations
The study had incomplete data due to low yield of BAL fluid and did not allow for definitive conclusions regarding the predictive value of prostanoid levels.
Participant Demographics
18 CDH patients (13 conventionally ventilated, 5 treated with ECMO) and 13 age-matched controls without pulmonary abnormalities.
